The Social Information System card (SIS card)

The card called SIS is the identification card of insured people. The SIS card (SIS = Social Information System) is a chip card of a credit card size storages the rights of the cardholder (or his assigns, children or spouse) covered by a social security agency of health expenditure.

What is it for?

Since 1998, the SIS card replaces the insurance card in the field of health care. It serves first and foremost to identify the insured people. Each worker or person entitled to a replacement income (unemployment, career breaks, disability, etc.) shall be in possession of his identity card in his social activities and trips. It is to be presented at the request of officials of the Social Security (medical care, workers compensation, family allowances, etc.) and tax officials. In this way, they can work faster. Do not worry: in practice, the SIS card is mainly used in hospitals, at the health care insurance agency pharmacist for mutuality and to issue you care, services or products as necessary.

The SIS card is personal. This is why each member of a household gets his/her own card which contains personal data only and does not mention any medical data.

Which information is stored in the SIS card?

The following identification data are in the SIS card:

  • surname

  • given name

  • gender

  • birth date

  • identification number of Social Security

  • card number

  • the start date of the validity of the card.

The memory of the SIS card also contains useful information for your insurance, your pharmacist or the hospital where you go for consultation or for a stay. These are data on your administrative status under the compulsory medical care insurance.

Functions of the SIS card

The SIS card has two main functions:

  • identify the insured person,

  • allow the insured person to enforce his rights and enforce obligations in connection with the application of social security.

Any change in the situation of the person or the loss of the card must be reported to the health care insurer.

The health care stickers are not removed. They are necessary and should be pasted on various health care documents delivered by GP, physiotherapists and other healthcare providers.

For further information, please contact your health care insurer.
